Out-of-Network Superbills
While I do not bill insurance directly, I am happy to provide you with a detailed "Superbill." You can submit this document to your insurance company to request out-of-network reimbursement, depending on your plan's benefits.

Letters, reports, and other documentation are $25 per document.
